Area of Science:

  • Obstetrics and Gynecology
  • Maternal Health
  • Epidemiology

Context:

  • Preeclampsia and eclampsia are major causes of maternal morbidity and mortality.
  • Historically, these conditions posed significant risks in the Czech Republic until the late 1970s.

Purpose:

  • To analyze causal links in maternal deaths related to severe preeclampsia and eclampsia.
  • To identify key errors in clinical management and risk factors.
  • To provide data on the frequency and characteristics of affected patient groups.

Summary:

  • A retrospective study analyzed 31 maternal deaths linked to severe preeclampsia and eclampsia in the Czech Republic (1978-2000).
  • Mortality rates decreased from 1978-1990 to 1991-2000.
  • Inadequate clinical management (48%) and delayed interventions were noted, with most deaths occurring postpartum.

Impact:

  • Improved diagnostic and therapeutic measures, including neonatology, have led to positive outcomes in recent years.
  • Focusing on early recognition and treatment of HELLP syndrome is essential.
  • Timely termination of pregnancies when preeclampsia symptoms worsen is now standard practice.
