Background:

  • Refractory obsessive-compulsive disorder (OCD) poses significant challenges to treatment.
  • Limited therapeutic options exist for patients with severe, treatment-resistant OCD.

Purpose of the Study:

  • To prospectively evaluate the efficacy of stereotactic bilateral anterior cingulotomy in patients with refractory OCD.
  • To assess the cognitive adverse effects associated with this neurosurgical intervention over a 12-month period.

Main Methods:

  • 14 patients with severe OCD, unresponsive to prior treatments, underwent stereotactic bilateral anterior cingulotomy.
  • Efficacy was measured using the Yale-Brown Obsessive-Compulsive Scale (Y-BOCS) and Clinical Global Impression (CGI).
  • Neuropsychological tests were administered pre- and post-operatively to evaluate cognitive changes.
