Background:

  • Sinus node dysfunction (SND) is an infrequent cause of bradycardia in children without structural heart disease.

  • ECG and 24-h Holter monitoring revealed atrioventricular junctional rhythms with rates as low as 24 bpm and asystole up to 3.7 seconds.

Findings:

  • The child initially received a VVIR pacemaker for symptom relief.
  • Over four years, lead failure and subsequent atrioventricular block necessitated a second pacemaker implantation via a subclavicular approach.
