    Summary

    Pindolol, a beta-blocking agent, proved beneficial in a challenging heart resuscitation case. It helped achieve a permanent sinus rhythm when defibrillation and lidocaine therapy were insufficient.

    Background:

    • Post-operative cardiac arrhythmias pose a significant risk.
    • Ventricular tachycardia and fibrillation are life-threatening arrhythmias requiring immediate intervention.

    Observation:

    • A 65-year-old female patient developed refractory ventricular tachycardia and fibrillation after complex abdominal aortic aneurysm surgery.
    • Standard resuscitation efforts including 20-25 DC-shocks and 1100 mg lidocaine over 50 minutes were initially ineffective.

    Findings:

    • Intravenous administration of pindolol (0.2 mg twice) was successful in converting the persistent arrhythmia.
    • Pindolol, as an adjunct to defibrillation and lidocaine, restored a permanent sinus rhythm.

    Implications:

    • Pindolol may be a valuable therapeutic option for refractory cardiac arrhythmias during resuscitation.
