Colonic polypoid disease: need for total colonoscopy.

    Summary

    Colonoscopic examination is crucial for suspected benign polypoid disease, as 19% of patients had unsuspected adenomas. Even with benign radiographic findings, colonoscopy can reveal significant neoplastic lesions, including cancer.

    Background:

    • Radiographic imaging is often used to detect polypoid disease in the colon.
    • Benign findings on imaging do not always exclude the presence of neoplastic lesions.

    Purpose of the Study:

    • To evaluate the diagnostic yield of colonoscopy in patients with radiographically suspected benign polypoid disease.
    • To determine the incidence of unsuspected neoplastic lesions in patients undergoing colonoscopy for suspected benign colonic polyps.

    Main Methods:

    • Colonoscopic examination of the entire colon was performed on 146 patients.
    • Pathologic documentation of all identified polypoid lesions was obtained.

    Main Results:

    • Of 36 patients with no neoplastic lesion at the suspected site, 7 (19%) had unsuspected adenomas elsewhere.
    • In 110 patients with a neoplastic lesion at the suspected site, 17 (15%) had adenocarcinomas or invasive carcinoma.
    • 128 additional unsuspected neoplastic polyps were found in 62 patients (56%), including 6 adenocarcinomas.

    Conclusions:

    • Colonoscopic evaluation is essential for suspected colonic polypoid disease, irrespective of radiographic findings.
    • Complete colonoscopic examination and pathologic documentation of all polyps are critical for accurate diagnosis and management.
