Background:

  • Intraductal papillary mucinous tumors (IPMNs) require accurate assessment of main pancreatic duct involvement.
  • Pancreatoscopy offers direct visualization but can be limited by papillary orifice stenosis.

Observation:

  • A mother-baby scope system was employed for pancreatoscopy.
  • Endoscopic sphincter dilation (ESD) using an Olbert balloon-tipped catheter was performed to facilitate scope insertion.
  • Direct visualization of tumorous lesions, including the characteristic salmon-roe appearance, was achieved in one patient.

Findings:

  • ESD successfully enabled smooth insertion of the endoscope into the main pancreatic duct.
  • Pancreatoscopy with ESD allowed for clear observation of tumorous lesions within the main pancreatic duct in one of two patients.
