    A diverting ileostomy effectively treated severe pseudomembranous colitis of unknown cause. The procedure allowed the colon to heal, enabling successful ileostomy closure and recovery.

    Background:

    • Pseudomembranous colitis (PMC) is an inflammation of the colon often associated with Clostridium difficile infection.
    • Severe or refractory cases may necessitate surgical intervention when medical management fails.

    Observation:

    • A patient presented with severe pseudomembranous colitis of unclear etiology.
    • The patient's condition was critical, requiring surgical consideration.

    Findings:

    • A completely diverting ileostomy resulted in a prompt and positive response.
    • The colon normalized within three months, allowing for successful ileostomy closure.

    Implications:

    • A diverting ileostomy is a viable and less extensive surgical option for severe PMC.
    • This approach may be preferable to more radical procedures in select cases.
