Summary

Familial angiotensin-converting enzyme (ACE) hyperactivity, characterized by significantly elevated ACE levels, is a genetic condition. This inherited trait, distinct from sarcoidosis, results from a gene mutation and typically presents without clinical symptoms.

Background:

  • Elevated serum angiotensin-converting enzyme (ACE) activity is often associated with sarcoidosis.
  • This study investigates individuals with extremely high ACE levels initially suspected to have sarcoidosis.

Observation:

  • Eight individuals presented with non-specific symptoms and markedly elevated serum ACE activity.
  • Sarcoidosis was ruled out in all patients.
  • Family history revealed similar high ACE levels in relatives, indicating a familial pattern.

  • A distinct familial disorder characterized by serum ACE levels 3-7 times the normal upper limit was identified.
  • This familial ACE hyperactivity is asymptomatic.
  • The condition is attributed to a point mutation in the ACE gene, leading to increased ACE shedding.
