Measuring adrenal function in out-patients using topical corticosteroids.

C G Sparkes

    The British Journal of Dermatology

    Summary

Topical corticosteroids generally do not affect pituitary adrenal function in outpatients. However, potent topical corticosteroids used extensively may cause temporary adrenal suppression.

    Area of Science:

    • Dermatology
    • Endocrinology

    Background:

    • Topical corticosteroids are widely used for various skin conditions.
    • Concerns exist regarding their potential systemic effects, particularly on the hypothalamic-pituitary-adrenal (HPA) axis.

    Purpose of the Study:

    • To review existing studies on the effects of topical corticosteroids on pituitary adrenal function.
    • To assess the risk of adrenal suppression in patients using these medications.

    Main Methods:

    • Systematic review of published literature.
    • Analysis of studies investigating topical corticosteroid use and adrenal function markers.

    Main Results:

    • Most outpatients using correctly prescribed topical corticosteroids show minimal impact on adrenal function.
    • Significant use of potent topical corticosteroids for extensive skin disease may lead to transient adrenal suppression.

    Conclusions:

    • Topical corticosteroid therapy is generally safe for most outpatients regarding adrenal function.
    • Clinicians must remain vigilant for potential HPA axis suppression in specific high-risk patient groups.
