Summary

Elective cesarean delivery for term breech presentation is associated with lower rates of infection and hemorrhage compared to emergency cesarean. However, it shows a higher rate of puerperal fever than vaginal delivery, with overall low severe maternal complication risks.

Area of Science:

  • Obstetrics and Gynecology
  • Maternal-Fetal Medicine
  • Epidemiology

Background:

  • Term breech presentation management involves delivery decisions with potential maternal risks.
  • Comparing elective cesarean delivery (CD) with vaginal delivery (VD) and emergency CD is crucial for optimizing maternal outcomes.

Purpose of the Study:

  • To compare maternal complications of elective CD for term breech with those of vaginal or emergency CD.

Main Methods:

  • Population-based, retrospective cohort study.
  • Included 15,441 primiparas delivering singleton breech at term.
  • Data sourced from Danish national health registers.

Main Results:

  • Elective CD had lower rates of puerperal fever, pelvic infection, hemorrhage, anemia, and wound infection operations than emergency CD.

Related Experiment Videos

  • Elective CD had a higher rate of puerperal fever and pelvic infection compared to VD.
  • Thromboembolic disease (0.1%) and anal sphincter rupture (1.7%) were noted for CD and VD, respectively.
  • No increased risk of subsequent ectopic pregnancy, miscarriage, placental complications, uterine rupture, or adverse neonatal outcomes with elective CD.
  • Conclusions:

    • Elective cesarean delivery for term breech presentation is associated with a low risk of severe maternal complications.
    • The study provides evidence for informed decision-making in term breech delivery management.
