Background:

  • Modern cataract surgery aims for immediate visual rehabilitation.
  • Achieving the desired refractive outcome shortly after surgery is a key goal.

Purpose of the Study:

  • To evaluate the refractive stability and accuracy after cataract surgery using different intraocular lens (IOL) types.
  • To compare the refractive results with the intended preoperative correction.

Main Methods:

  • Phacoemulsification with scleral incision and PMMA IOL implantation (104 patients).
  • Temporal clear cornea incision with foldable IOL (Acrysof) implantation (70 patients).
  • Refraction and astigmatism measured preoperatively, 1-3 days postoperatively, and at least 5 months later.

Main Results:

  • Within 1-3 days post-op, 73-77% of patients had a refractive error within +/- 1.0 D of target.
  • At 5 months, 76-86% were within +/- 1.5 D and 96-100% within +/- 2.0 D.
  • Significant refractive changes (>1.0 D) occurred between early and late post-op periods in 16-20% of patients.

Conclusions:

  • The goal of immediate, stable refractive results in cataract surgery has not been fully achieved.
  • Refractive changes observed in both scleral and clear cornea incision techniques preclude immediate postoperative glasses prescription.
  • Further advancements are needed to ensure predictable and stable refractive outcomes after cataract surgery.
