Summary

In dogs with lymphoma, urine cortisol:creatinine ratios are often abnormal, potentially indicating chronic illness rather than hyperadrenocorticism. Other tests like plasma cortisol and adrenal width may be more specific for hyperadrenocorticism.

Background:

  • Hyperadrenocorticism (HAC) is often suspected in dogs with chronic illness due to biochemical abnormalities.
  • Lymphoma (LSA) is a common chronic disease in dogs that can present with similar biochemical changes.
  • Evaluating the specificity of screening tests for HAC in dogs with LSA is crucial for accurate diagnosis.

Purpose of the Study:

  • To assess the diagnostic specificity of adrenocortical screening tests in dogs diagnosed with lymphoma.
  • To determine if commonly used tests for HAC are reliable in dogs with LSA.

Main Methods:

  • Included ten dogs with LSA, excluding those with prior HAC diagnosis or glucocorticoid treatment.
  • Collected serial plasma cortisol concentrations (PACs) post-ACTH stimulation, urine cortisol:creatinine (UC:Cr) ratios, and maximal left adrenal width measurements.

    • Urine cortisol:creatinine (UC:Cr) ratios were frequently abnormal (26/36), with 90% of dogs showing at least one abnormal result.
    • Post-ACTH stimulation plasma cortisol concentrations (PACs) and maximal left adrenal width measurements were predominantly normal.
    • Only one PAC was abnormal, and no adrenal width measurements were abnormal.

    Conclusions:

    • The UC:Cr ratio appears sensitive but potentially not specific for HAC in dogs with LSA, possibly reflecting chronic illness-related cortisol increases.
    • Plasma cortisol concentrations and adrenal width measurements may be more specific for HAC or less sensitive to chronic illness-induced cortisol changes.
