Volvulus of the small bowel.

L J De Souza

    Summary

High intake of local "kongo" beer in Uganda was linked to a significant number of small bowel volvulus cases. Serotonin in the beer may be a contributing factor to this condition.

    Area of Science:

    • Gastroenterology
    • Toxicology
    • Public Health

    Background:

    • Intestinal obstruction is a common surgical emergency.
    • Primary small bowel volvulus is a rare cause of obstruction, particularly in certain geographic regions.
    • The role of dietary factors in volvulus formation requires further investigation.

    Purpose of the Study:

    • To investigate the incidence and potential causes of primary small bowel volvulus in northern Uganda.
    • To explore the association between local beverage consumption and the occurrence of small bowel volvulus.
    • To identify potential etiological agents contributing to volvulus.

    Main Methods:

    • Retrospective case series analysis of intestinal obstruction cases over two years.
    • Clinical data review focusing on small bowel volvulus and associated factors.
    • Analysis of the composition of a local beverage implicated in the cases.

    Main Results:

    • Twelve cases of primary small bowel volvulus were identified out of 65 non-herniated intestinal obstructions.
    • A high incidence of volvulus was correlated with substantial consumption of local "kongo" beer.
    • The implicated beer exhibited high serotonin concentrations; serotonin is a potential causative agent in 75% of cases.
    • Systemic symptoms of circulatory collapse were notably absent.

    Conclusions:

    • Local "kongo" beer consumption is associated with a high incidence of primary small bowel volvulus in this Ugandan population.
    • High serotonin levels in "kongo" beer may play a significant role in the pathogenesis of small bowel volvulus.
    • Further research is warranted to elucidate the mechanism of serotonin-induced volvulus and public health implications.
