Zosteriform Lichen planus.

Background:

Lichen planus (LP) is a common inflammatory condition affecting skin and mucous membranes.
LP can rarely present in a zosteriform pattern, typically in pre-existing herpes zoster scars.
Zosteriform LP without a history of herpes zoster is exceptionally rare.

Observation:

A patient presented with grouped lichenoid lesions unilaterally on the neck.
The patient had no history of herpes zoster (shingles) at the affected site or elsewhere.
This presentation occurred in the head and neck region, an area with limited prior reports for this specific LP variant.

Findings:

The case describes an extremely rare zosteriform pattern of lichen planus.
The lesions mimicked herpes zoster clinically but lacked any viral etiology.
This represents a novel presentation of lichen planus in the head and neck area.

Implications:

This case expands the known clinical spectrum of lichen planus.
Dermatologists should consider lichen planus in the differential diagnosis of zosteriform eruptions, even without a history of herpes zoster.
Awareness of such rare presentations is crucial for accurate diagnosis and management of skin conditions.
