Left axis deviation: a spectrum of intraventricular conduction block.

    Summary

Left axis shifts significantly prolong intraventricular conduction time (IVCT). This study reveals a continuous relationship between QRS axis deviation and IVCT, indicating a spectrum of conduction block.

    Area of Science:

    • Cardiology
    • Electrophysiology
    • Medical Diagnostics

    Background:

    • Left axis shift is a common electrocardiographic finding.
    • Its impact on intraventricular conduction time (IVCT) requires further elucidation.
    • Understanding this relationship aids in diagnosing conduction abnormalities.

    Purpose of the Study:

    • To investigate the effect of left axis shift on intraventricular conduction time (IVCT).
    • To determine the correlation between QRS axis deviation and IVCT.
    • To explore if left axis shift represents a spectrum of intraventricular conduction block.

    Main Methods:

    • Analysis of electrocardiograms from 63 subjects before and after left axis shift.
    • Exclusion of subjects with confounding cardiac conditions or medications.
    • Correlation analysis between QRS axis (AQRS) and IVCT.

    Main Results:

    • A significant average increase of 25 msec in IVCT was observed with left axis shifts (≥ -30 degrees).
    • A strong linear correlation (r = -0.8) was found between AQRS and IVCT.
    • Prolongation of IVCT occurred with all leftward AQRS shifts from +90 degrees.

    Conclusions:

    • Left axis shifts, including left anterior hemiblock, are associated with prolonged IVCT.
    • A continuum exists between QRS axis and IVCT, suggesting a spectrum of intraventricular conduction block.
    • Electrocardiographic axis deviation is a valuable indicator of intraventricular conduction abnormalities.
