Photochemotherapy (PUVA) for psoriasis may paradoxically induce lichen planus. This case suggests PUVA-induced cell injury could trigger an autoimmune response, leading to new skin lesions.

Area of Science:

  • Dermatology
  • Immunology
  • Photobiology

Background:

  • Psoriasis vulgaris is a chronic autoimmune skin condition.
  • Oral 8-methoxypsoralen and UVA (PUVA) therapy is a common treatment for psoriasis.

Observation:

  • A patient with psoriasis developed pruritic, violaceous papules on the forearm and thigh after 5 months of PUVA therapy.
  • Histopathology confirmed these lesions were consistent with lichen planus (LP).

Findings:

  • Lichen planus lesions appeared after 45 sittings of PUVA, with a cumulative dose of 156 J/cm(2).
  • The lesions resolved within one month of discontinuing PUVA therapy.

Implications:

  • PUVA therapy may induce lichen planus through cell injury, exposing sequestered antigens.
