The Duffy antigen-receptor for chemokines (DARC) is upregulated in renal allograft rejection. DARC expression increases with both cellular and humoral rejection, particularly when both occur simultaneously.

Area of Science:

  • Nephrology
  • Immunology
  • Transplantation

Background:

  • The Duffy antigen-receptor for chemokines (DARC) is a chemokine-binding protein.
  • DARC is upregulated on peritubular capillaries (PTC) during cellular renal allograft rejection.
  • C4d deposition and inflammatory cell accumulation in PTC indicate humoral renal allograft rejection.

Purpose of the Study:

  • To evaluate DARC expression in different forms of human renal allograft rejection.
  • To investigate the role of DARC in inflammatory cell recruitment at the site of C4d deposition.
  • To correlate DARC expression with C4d deposition and rejection types.

Main Methods:

  • Immunohistochemistry was used to assess C4d deposition and DARC expression in 42 renal transplant biopsies.

Related Experiment Videos

  • Biopsies were categorized into C4d-negative (with/without cellular rejection) and C4d-positive (humoral rejection, with/without cellular rejection) groups.
  • Quantification of DARC-positive PTC per high-power field (HPF) was performed.
  • Main Results:

    • DARC expression was minimal in non-rejected kidneys.
    • Both cellular and humoral rejection significantly increased DARC-positive PTC numbers (9.7 and 8.7 vessels/HPF, respectively) compared to controls (2.6 vessels/HPF).
    • Combined humoral and cellular rejection showed the highest DARC expression (17.5 vessels/HPF).

    Conclusions:

    • This study is the first to show DARC induction at C4d deposition sites in humoral allograft rejection.
    • The findings suggest that DARC expression is induced by rejection processes.
    • Additive effects of humoral and cellular rejection on DARC expression imply distinct induction pathways.
