Background:

  • Lichen planus (LP) is an inflammatory dermatosis often challenging to diagnose when exclusively affecting the female genitalia.
  • Vulvar LP frequently co-occurs with other immune-mediated disorders, necessitating multidisciplinary management by dermatologists and gynecologists.

Observation:

  • A 37-year-old woman presented with a 3-month history of vulvar lichen planus, characterized by diffuse whitish papules and plaques causing sclerosis and edema.
  • The patient had a history of ulcerative colitis since age 21, with no concurrent oral or skin lesions typical of lichen planus.
  • Histopathological examination confirmed classic features of lichen planus despite the mixed and hypertrophic clinical presentation.

Findings:

  • Diagnosis of isolated vulvar lichen planus was confirmed histologically.
