    Background:

    • Striking hyperammonemia was noted, but liver examination showed minimal fatty infiltration.

    • This case suggests that the hallmark massive fatty liver degeneration may not be a prerequisite for Reye's syndrome diagnosis.
    • The findings challenge the classical definition of Reye's syndrome.
