Summary

This case study highlights severe spontaneous echocontrast in the left atrium of a patient with apical hypertrophic cardiomyopathy and transient ischemic attack. This finding suggests a potential link between HCM, left atrial SEC, and ischemic events.

Area of Science:

  • Cardiology
  • Cardiovascular Imaging
  • Neurology

Background:

  • Transient ischemic attack (TIA) is a critical warning sign for stroke.
  • Apical hypertrophic cardiomyopathy (HCM) is a distinct form of cardiac hypertrophy.
  • Left atrial spontaneous echocontrast (SEC) is associated with thromboembolic risk.

Observation:

  • A 66-year-old male presented with TIA symptoms.
  • Transthoracic echocardiography identified apical HCM and sinus rhythm.
  • Transesophageal echocardiography revealed severe spontaneous echocontrast (SEC) within the left atrial (LA) cavity.

Findings:

  • The patient exhibited significant apical hypertrophy.
  • Severe SEC was observed in the left atrium, correlating with apical HCM.

Related Experiment Videos

  • The presence of SEC in the context of HCM and TIA warrants further investigation.
  • Implications:

    • This case underscores the importance of comprehensive echocardiographic evaluation in patients with HCM presenting with TIA.
    • Left atrial SEC may represent a significant risk factor for embolic events in apical HCM.
    • Further research is needed to elucidate the pathogenic mechanisms linking apical HCM, LA SEC, and cerebrovascular events.
