Endoscopic sinus surgery (ESS) can rarely cause ischemic optic neuropathy (ION), leading to vision loss. Prompt ophthalmological consultation and supportive care are crucial for managing this rare complication.

  • A 51-year-old male patient experienced vision loss after an otherwise uncomplicated ESS.
  • Ischemic optic neuropathy (ION) presents as acute or subacute postoperative vision loss.
  • Risk factors for ION include intraoperative anemia, hypotension, and systemic conditions like hypertension and diabetes.

Findings:

  • This case highlights a previously unreported visual complication of ESS: ischemic optic neuropathy (ION).
