Area of Science:

  • Medical Microbiology
  • Infectious Diseases
  • Pulmonology

Background:

  • Rhodococcus equi is an opportunistic pathogen that can cause severe pneumonia, particularly in immunocompromised individuals.
  • Human Immunodeficiency Virus (HIV) infection significantly increases susceptibility to opportunistic infections.
  • Campania, Italy, has not previously reported cases of Rhodococcus equi pneumonia in HIV-positive patients.

Purpose of the Study:

  • To document the first occurrence of Rhodococcus equi pneumonia in an HIV-positive patient in the Campania region.
  • To describe the clinical presentation, diagnostic findings, and treatment response.
  • To highlight the challenges in managing pulmonary lesions caused by this pathogen in immunocompromised hosts.

Main Methods:

  • Case report detailing a single patient's clinical course.

Related Experiment Videos

  • Review of medical records, including diagnostic imaging (radiography, CT scans).
  • Assessment of microbiological data and antibiotic susceptibility testing.
  • Clinical and radiological follow-up over 10 months.
  • Main Results:

    • Successful resolution of clinical symptoms following targeted antibiotic therapy.
    • Persistence of pulmonary lesions, with only partial reduction in size observed over 10 months of radiological follow-up.
    • Confirmation of Rhodococcus equi as the causative agent.

    Conclusions:

    • Rhodococcus equi pneumonia is a potential opportunistic infection in HIV-positive individuals in Campania.
    • While antibiotic therapy can effectively manage clinical symptoms, radiological resolution of pulmonary lesions may be prolonged.
    • This case underscores the importance of considering atypical pathogens in immunocompromised patients with respiratory symptoms.
