Background:

Sphenoethmoidal encephalocele is a rare congenital malformation.
Early diagnosis and surgical intervention are crucial for favorable outcomes.

Observation:

A neonate presented with a large sphenoethmoidal encephalocele.
The encephalocele extended through the skull base into the nasopharynx.

Findings:

Surgical management involved an extracranial, transpalatal approach.
Complete excision of the encephalocele was achieved with no immediate complications.

Implications:

This case highlights the efficacy of the transpalatal approach for sphenoethmoidal encephaloceles.
Successful treatment can prevent significant neurological and cosmetic sequelae.
