Summary

Autogenous fat injection effectively treats vocal fold atrophy with complete glottal closure (VFACGC), improving voice quality and function. This procedure also resolved muscular tension dysphonia in over half of the patients studied.

Background:

  • Autogenous fat augmentation is a known treatment for glottic insufficiency.
  • Limited data exists on its efficacy for vocal fold atrophy with complete glottal closure (VFACGC).

Observation:

  • This retrospective study evaluated 21 patients with VFACGC undergoing fat injection.
  • 13 patients had pre- and post-procedure assessments of perceptual, acoustic, phonatory, and videolaryngostroboscopic functions.
  • Mean follow-up was 9.5 months.

Findings:

  • Excellent results were observed in 15 patients; four experienced failure.
  • Significant improvements were noted in perceptual voice ratings (grade, roughness, breathiness) and videolaryngostroboscopic parameters (vocal fold edge linearity, vibration amplitude, mucosal wave).

  • 54% of patients with co-existing muscular tension dysphonia (MTD) experienced MTD resolution post-procedure.
  • Implications:

    • Fat injection is an effective autogenous implant for VFACGC, often misdiagnosed as MTD.
    • It offers significant functional voice improvement and can resolve associated MTD.
    • Repeat procedures may mitigate fat re-absorption issues.
