Summary

Visual evoked potential (VEP) normalization in optic neuritis (ON) is rare. While some patients may experience temporary VEP latency recovery, it often deteriorates due to recurrent demyelination.

Background:

  • Optic neuritis (ON) can cause visual evoked potential (VEP) abnormalities.
  • Normalization of VEP latency in ON patients is infrequently reported, with limited follow-up data.

Purpose of the Study:

  • To investigate the long-term normalization of P2 wave latency in the affected eye of patients with acute unilateral ON.
  • To assess the duration of normal VEP latency if it occurs and the factors influencing it.

Main Methods:

  • 18 patients with acute unilateral ON underwent VEP testing for 5 years.
  • A control group of 18 healthy subjects established the normal P2 latency range (upper limit: 115.9 ms).
  • VEP P2 wave latency was measured in affected eyes at baseline and during follow-up.

  • Mean P2 latency at presentation was 140 ms (SD 16 ms).
  • VEP latencies generally remained stable over 5 years.
  • Two patients showed temporary normalization of P2 latency, which became prolonged again within 2 years.

Conclusions:

  • Delayed P2 latency in ON can normalize in a small subset of patients.
  • This normalization may be transient, with potential relapse due to subacute demyelination.
