Area of Science:

  • Hepatology
  • Infectious Diseases
  • Immunology

Background:

  • Hepatitis A infection poses a significant public health concern.
  • Post-exposure prophylaxis is crucial for preventing disease transmission.
  • Evidence on optimal prophylaxis strategies requires synthesis.

Purpose of the Study:

  • To identify the most effective post-exposure prophylaxis (PEP) for Hepatitis A.
  • To determine the longest effective interval for PEP administration.
  • To evaluate the efficacy of current immunoglobulin treatments for Hepatitis A.

Main Methods:

  • Exploratory review of English-language publications from 1945 onwards.
  • Focused on identifying studies detailing post-exposure prophylaxis for Hepatitis A.
  • Assessed evidence regarding immunoglobulin efficacy and optimal timing.

Main Results:

  • No evidence found supporting the efficacy of post-exposure immunoglobulin administration in preventing Hepatitis A infection.
  • Current data is insufficient to establish an effective interval for immunoglobulin prophylaxis.
  • Active immunization is recommended for secondary contacts.

Conclusions:

  • Widespread recommendation of immunoglobulins for Hepatitis A immunoprophylaxis is not supported by current evidence.
  • A systematic review is necessary to definitively assess immunoglobulin efficacy.
  • Prioritizing active immunization for contacts of exposed individuals is advised.
