Summary

Severe bacterial orbital cellulitis can threaten vision. Extended pterional orbital decompression effectively reduced orbital pressure and improved vision in three patients, offering a valuable surgical option.

Background:

  • Bacterial orbital cellulitis is a rare infection that can compromise orbital structures.
  • Standard antibiotic therapy may be insufficient for severe cases.
  • Sinus surgery is sometimes required for drainage.

Observation:

  • Three patients with severe orbital cellulitis presented with vision loss, proptosis, pupillary disturbances, and restricted eye movements despite antibiotics.
  • These patients underwent extended pterional orbital decompression, involving removal of lateral and superolateral orbital walls to reduce intraorbital pressure.

Findings:

  • All three patients showed significant improvement in their presenting symptoms following the surgical decompression.
  • No operative complications were reported in any of the cases.
