Area of Science:

  • Infectious Diseases
  • Otolaryngology
  • Dermatology

Background:

  • Syphilis is a complex systemic infection known for its diverse clinical presentations.
  • Cervical lymphadenopathy is a common clinical finding with a broad differential diagnosis.
  • Traditionally, syphilis is not widely recognized as a primary cause of isolated cervical lymphadenopathy.

Observation:

  • A 21-year-old male presented with significant unilateral cervical lymphadenopathy.
  • The patient lacked typical syphilitic markers such as oropharyngeal chancres or genital/skin lesions.
  • Imaging studies, including CT and MR, revealed extensive cervical lymphadenopathy.

Findings:

  • Serologic tests for syphilis were positive in the patient.
  • The clinical presentation and diagnostic findings strongly implicated syphilis.

Related Experiment Videos

  • This case demonstrates a less common manifestation of secondary syphilis.
  • Implications:

    • Clinicians should consider syphilis in the differential diagnosis of cervical lymphadenopathy, particularly in young adults.
    • The "great mimicry" of syphilis extends to presentations without classic mucocutaneous signs.
    • Early recognition and treatment of syphilitic cervical lymphadenopathy are crucial to prevent complications.
