Background:

  • Spontaneous iliac artery dissection (SIAD) is an uncommon yet significant vascular emergency.
  • High-performance athletes may present with exertional leg pain mimicking musculoskeletal issues.

Observation:

  • A 60-year-old competitive cyclist developed acute claudication after intense exercise.
  • Angiography revealed a dissection flap in the right common and external iliac arteries.

Findings:

  • Initial endovascular treatment with an uncovered stent failed to resolve the false lumen.
  • Successful management required subsequent open surgical intervention.

Implications:

  • SIAD should be considered in athletes presenting with exercise-induced leg pain.
