Summary

Systemic contact dermatitis from nickel can arise from direct skin contact with metal objects, not just internal exposure. Avoiding nickel sources and treatment led to symptom improvement in patients.

Background:

  • Systemic contact dermatitis (SCD) is typically associated with internal nickel exposure routes.
  • Local nickel exposure has been less commonly recognized as a trigger for SCD.

Purpose of the Study:

  • To investigate the role of continual local skin contact with nickel in eliciting systemic contact dermatitis.
  • To identify the source and characteristics of nickel-induced SCD in affected patients.

Main Methods:

  • Case study of 20 patients with systemic contact dermatitis.
  • Patch testing with nickel sulfate and dimethylglyoxime test for nickel detection on clothing accessories.
  • Clinical observation of dermatitis spread and response to treatment.

Main Results:

  • All patients presented with periumbilical dermatitis due to nickel-containing belt buckles/buttons, which spread to other body sites.
  • Positive patch tests to nickel sulfate and detection of free nickel on metal accessories confirmed nickel allergy.
  • Avoidance of nickel exposure and standard dermatitis treatment resulted in significant clinical improvement.

Conclusions:

  • Continual local skin contact with nickel can be a significant trigger for systemic contact dermatitis.
