Background:

  • Bird fancier's lung (BFL) is a hypersensitivity pneumonitis triggered by avian antigens.
  • Diagnosis of BFL from feathers is challenging as feathers are not commonly recognized as causative agents.
  • Chronic BFL can present as pulmonary fibrosis.

Observation:

  • A 73-year-old woman with exertional dyspnea over three years.
  • History of pigeon exposure and prolonged use of a feather duvet.
  • Radiographic findings included peripheral reticular infiltrates and HRCT showing consolidation, micronodules, and ground-glass opacities.

Findings:

  • Positive lymphocyte proliferation to feather antigen and a positive bird antigen inhalation provocation test.
  • Histopathology consistent with chronic hypersensitivity pneumonitis.
