    Summary

    Knee fusion surgery often leads to complications, with persistent pain being most common. Due to advancements in total knee replacement, knee fusion is now best reserved for chronic infections or failed joint replacements.

    Background:

    • Knee fusion (arthrodesis) has historically been used for severe knee problems.
    • This review examines outcomes following knee fusion, primarily for degenerative arthritis and sepsis.

    Observation:

    • A review of 41 patients undergoing knee fusion revealed a high complication rate, with 85% experiencing adverse events.
    • Complications included persistent knee and back pain, impacting patients' return to work and social activities.

    Findings:

    • The optimal position for knee fusion is 15-20 degrees of flexion to improve gait and car driving.
    • Persistent pain was the most frequent early and late complication after knee fusion.

    Implications:

    • With improved total knee replacement (TKR) techniques, knee arthrodesis indications are narrowing.
    • Knee fusion should be reserved for cases of chronic knee infection or following failed TKR.
