Abstract:
The Department of Health wants to turn home improvement agencies into key players in provision of services to older people discharged from hospital. 104 clients have been referred to the project: 55 related to hospital discharge and 49 to admission prevention. Having a single point of contact for a range of home improvement services has made referrals easier.
