Background:

Trabeculectomy is a common glaucoma surgery.
Circumferential conjunctival blebs are an uncommon complication of trabeculectomy.
Symptomatic blebs can cause discomfort and affect vision.

Observation:

A novel surgical technique, bleb-limiting conjunctivoplasty, was performed on four eyes of two patients.
The procedure involved radial conjunctival and Tenon incisions sutured to the sclera.
A corneal traction suture facilitated conjunctival exposure.

Findings:

Immediate flattening of the conjunctiva and no elevation in intraocular pressure were observed postoperatively.
Patients reported rapid symptom improvement with no immediate complications.

Implications:

Bleb-limiting conjunctivoplasty appears to be an effective treatment for symptomatic circumferential blebs post-trabeculectomy.
This technique offers a potential solution for managing this specific complication.
