Temporal arteritis can present with nonspecific orbital inflammation, causing vision loss and proptosis. External beam radiation therapy effectively resolved these symptoms in an elderly patient.

Area of Science:

  • Ophthalmology
  • Rheumatology
  • Oncology

Background:

  • Temporal arteritis (Giant Cell Arteritis) is a systemic vasculitis primarily affecting medium and large arteries.
  • Ocular involvement in temporal arteritis can lead to severe visual impairment, including blindness.
  • Orbital inflammation is an uncommon manifestation of temporal arteritis.

Observation:

  • A 75-year-old woman presented with acute vision loss, periocular pain, optic disc edema, proptosis, and an intraconal mass.
  • Initial investigations, including sedimentation rate, did not suggest temporal arteritis.
  • Orbital biopsy revealed chronic granulomatous inflammation with perivasculitis.

Findings:

  • Temporal artery biopsy confirmed findings consistent with temporal arteritis.

Related Experiment Videos

  • Despite nonspecific initial presentation, the underlying cause was identified as temporal arteritis.
  • The patient received 2000 cGy of external beam radiation therapy.
  • Implications:

    • This case highlights that temporal arteritis can present with atypical orbital findings.
    • Pathologic findings in temporal arteritis may not always be specific.
    • External beam radiation therapy demonstrated efficacy in managing this unusual presentation of orbital inflammation associated with temporal arteritis.
