Povidone-iodine (PI) pretreatment harms small bowel storage quality by increasing injury and permeability. This method offers no additional benefit for intestinal decontamination during organ preservation.

Area of Science:

  • Transplantation research
  • Organ preservation techniques
  • Surgical decontamination

Background:

  • Povidone-iodine (PI) is utilized for duodenal decontamination in pancreas transplantation.
  • Current use aims to prevent intestinal complications.

Purpose of the Study:

  • To evaluate the impact of luminal PI pretreatment combined with amino acid (AA)-rich solutions on small bowel (SB) storage.
  • To determine if PI enhances decontamination efficacy or affects SB preservation quality.

Main Methods:

  • Rat small bowels were preserved using University of Wisconsin solution.
  • Luminal administration involved combinations of AA solution with or without PI pretreatment.
  • Groups received varying volumes of AA and PI solutions.
  • Analysis included cellular energetics, permeability, and histology over 10-hour cold storage.

Related Experiment Videos

Main Results:

  • PI treatment significantly reduced adenosine triphosphate (ATP) levels.
  • Mannitol permeability increased in PI-treated groups, indicating greater injury.
  • Histologic injury scores were higher in PI-treated small bowels.
  • Both PI and AA solutions effectively cleared over 95% of enteric bacteria.

Conclusions:

  • Short-term luminal exposure to PI exacerbates small bowel injury during preservation.
  • PI pretreatment provides no added benefit for intestinal decontamination beyond standard solutions.
  • The findings suggest PI is detrimental to small bowel graft quality.
