Insights
Myocardial revascularization without extracorporeal circulation is a reproducible technique for ischemic heart disease. Graft patency was confirmed postoperatively, showing its viability as an alternative treatment.
Area of Science:
- Cardiovascular Surgery
- Cardiac Surgery
- Minimally Invasive Cardiac Surgery
Background:
- Ischemic heart disease remains a leading cause of mortality worldwide.
- Traditional myocardial revascularization often involves extracorporeal circulation (ECC).
- Off-pump coronary artery bypass (OPCAB) surgery offers an alternative to ECC, potentially reducing complications.
Purpose of the Study:
- To evaluate the immediate postoperative outcomes of patients undergoing myocardial revascularization without extracorporeal circulation (off-pump coronary artery bypass - OPCAB).
- To assess the efficacy and safety of different graft types used in OPCAB surgery.
- To identify potential complications and management strategies in the early postoperative period.
Main Methods:
- A prospective study involving 112 patients undergoing OPCAB surgery.
- Patients were selected based on specific criteria including coronary artery caliber, lack of intramyocardial trajectory, noncalcified arteries, and tolerance to maneuvers.
- Postoperative monitoring included clinical observation and coronary angiography on the seventh postoperative day.
Main Results:
- OPCAB was successfully performed in 112 patients, with 3 requiring conversion to ECC (no impact on mortality).
- Common intraoperative events included arterial hypotension (89 patients), ventricular extrasystoles (58 patients), and atrial fibrillation (10 patients).
- Coronary angiography confirmed graft patency in all assessed patients on postoperative day 7.
Conclusions:
- Myocardial revascularization without extracorporeal circulation (OPCAB) is a reproducible and safe technique.
- OPCAB serves as a viable alternative for treating patients with ischemic heart disease.
- The study highlights the feasibility and positive graft outcomes associated with OPCAB surgery.
Objective:
To assess the immediate postoperative period of patients undergoing myocardial revascularization without extracorporeal circulation with different types of grafts.
Methods:
One hundred and twelve patients, 89 (79.5%) of whom were males, were revascularized without extracorporeal circulation. Their ages ranged from 39 to 85 years. The criteria for indicating myocardial revascularization without extracorporeal circulation were as follows: revascularized coronary artery caliber > 1.5 mm, lack of intramyocardial trajectory on coronary angiography, noncalcified coronary arteries, and tolerance of the heart to the different rotation maneuvers.
Results:
Myocardial revascularization without extracorporeal circulation was performed in 112 patients. Three were converted to extracorporeal circulation, which required a longer hospital stay but did not impact mortality. During the procedure, the following events were observed: atrial fibrillation in 10 patients, ventricular fibrillation in 4, total transient atrioventricular block in 2, ventricular extrasystoles in 58, use of a device to retrieve red blood cells in 53, blood transfusion in 8, and arterial hypotension in 89 patients. Coronary angiography was performed in 20 patients on the seventh postoperative day when the grafts were patent.
Conclusion:
Myocardial revascularization without extracorporeal circulation is a reproducible technique that is an alternative for treating ischemic heart disease.
