Background:

Observation:

  • Five patients presented with acute vision loss and metamorphopsia over 2 weeks to 3 months.
  • The condition was secondary to macular pucker following retinal tears or detachment.
  • All patients underwent vitrectomy and epiretinal membrane removal within one month of diagnosis.

Findings:

  • Early surgical intervention resulted in rapid visual acuity recovery and resolution of metamorphopsia within 6 weeks to 3 months.
  • Histopathology revealed tubuloacinar structures and increased retinal pigment epithelium-derived cells in these cases compared to idiopathic macular pucker.
  • Surgical complications in two patients impacted the final visual outcome.

Implications:

  • Acute macular pucker appears to be a distinct clinicopathologic entity.
  • Prompt surgical management is recommended for favorable visual outcomes.
  • Further research into the specific cellular mechanisms driving this condition is warranted.
