Background:

Adrenoleukodystrophy (ALD) is a rare genetic peroxisomal disorder.
ALD is characterized by impaired very long-chain fatty acid (VLCFA) metabolism, leading to adrenal insufficiency and demyelination.

Observation:

The first reported case of ALD in a Chinese boy presented with hyperpigmentation, behavioral changes, and brain demyelination on MRI.
Biochemical analysis confirmed elevated serum VLCFAs and adrenal insufficiency.

Findings:

Dietary management with very long-chain fatty acid restriction and glyceryl trierucate/glycerol trioleate oil (GTEO, Lorenzo's oil) did not halt disease progression.
The patient experienced worsening visual loss and spastic tetraparesis despite GTEO, adrenal steroid replacement, and intravenous immunoglobulin therapy.
