Summary

New research reveals previously undescribed glomerular lesions in sarcoidosis patients caused by calcium deposits. These findings in kidney biopsies link calcium deposition to kidney dysfunction in sarcoidosis.

Background:

  • Sarcoidosis is a multisystem inflammatory disease.
  • Renal involvement in sarcoidosis commonly includes interstitial granulomatous nephritis and nephrocalcinosis.
  • Glomerular lesions associated with sarcoidosis have not been previously documented.

Observation:

  • Five renal biopsy specimens from four patients with sarcoidosis were analyzed.
  • Light, electron, and immunofluorescence microscopy were employed.
  • Interstitial granulomatous nephritis and nephrocalcinosis were present in all cases.

Findings:

  • Segmental glomerular lesions were observed in three patients.
  • These lesions showed thickened, wrinkled glomerular capillary walls and altered basement membranes.
  • Electron microscopy revealed calcific microspherules within the glomerular basement membrane, paramesangial zone, and mesangium.

Implications:

  • The structural damage from glomerular calcinosis may contribute to renal function abnormalities in sarcoidosis.
  • This study highlights a novel mechanism of kidney damage in sarcoidosis.
  • Further research is needed to understand the clinical significance and management of these glomerular findings.
