Background:

Laryngeal chondroradionecrosis is a delayed complication following radiotherapy for laryngeal tumors.
It can occur many years after treatment completion.

Observation:

A 78-year-old man presented with a prelaryngeal mass and hemoptysis 17 years post-radiotherapy for vocal cord carcinoma.
The lesion evolved into a cutaneous fistula, exposing the larynx.

Findings:

Post-radiotherapy laryngeal necrosis is a serious, albeit uncommon, complication.
Delayed presentation with fistula formation indicates extensive tissue damage.

Implications:

Exclusion of tumor recurrence is critical in managing such cases.
