Summary

Metaplastic synovial cysts, skin lesions resembling synovium, can occur without prior trauma in rheumatoid arthritis patients. Chronic inflammation and joint pressure may contribute to their development.

Background:

  • Metaplastic synovial cyst of the skin is a rare intradermal lesion.
  • Typically associated with prior surgical trauma.
  • Histology shows cystic structures with synovium-like lining.

Observation:

  • Two rheumatoid arthritis patients (46 and 55 years) presented with cystic nodules.
  • Lesions were located on the thumb and great toe.
  • No history of trauma or surgery in the affected areas.

Findings:

  • Vimentin and CD 68 positivity in cyst lining cells.
  • Supports similarity between metaplastic and normal synovium.
  • Suggests alternative pathogenetic mechanisms.
