Summary

Developmental venous anomalies (DVAs) may contribute to the development and recurrence of cryptic vascular malformations (CVMs). Occluding a DVA associated with a CVM resolved seizures and neurological deficits in a patient.

Background:

  • Developmental venous anomalies (DVAs) are common, but their clinical significance, particularly in relation to cryptic vascular malformations (CVMs), is debated.
  • Venous outflow disturbance and hypertension within DVAs are increasingly suspected as factors in CVM development.

Observation:

  • A 32-year-old man presented with recurrent, rebleeding CVM in the left angular gyrus, draining into a large DVA.
  • The CVM recurred after initial surgical removal of a cavernoma, with the DVA left intact.
  • A second surgery involved removing a newly formed arteriovenous angioma (AVM) and occluding the associated DVA.

Findings:

  • Histological verification confirmed the recurrent lesion as an AVM.
  • Post-occlusion, the patient experienced no seizures and remained neurologically intact.
