Summary

Patients with periampullary diverticulum face greater challenges during common bile duct cannulation in ERCP. This condition also increases the risk of retained stones, potentially leading to more post-ERCP complications.

Area of Science:

  • Gastroenterology
  • Endoscopic Procedures
  • Biliary Tract Diseases

Background:

  • Periampullary diverticula are often found incidentally during ERCP.
  • While usually asymptomatic, they can cause significant patient morbidity.
  • This study investigates ERCP in patients with periampullary diverticulum.

Purpose of the Study:

  • To evaluate ERCP indications in patients with periampullary diverticulum.
  • To assess the procedural course and outcomes of ERCP in this patient group.
  • To analyze post-ERCP complications in patients with and without periampullary diverticulum.

Main Methods:

  • Retrospective analysis of clinical, laboratory, and imaging data.
  • Inclusion of 626 patients from a single endoscopy center.

Related Experiment Videos

  • Focus on patients undergoing ERCP, comparing those with and without periampullary diverticulum.
  • Main Results:

    • Periampullary diverticulum identified in 11.5% of patients.
    • Significantly higher rates of papilla of Vater cannulation difficulty (79.2% vs. 9.7%) and common bile duct cannulation failure (11.1% vs. 4.7%) in patients with diverticula.
    • Patients with diverticula showed a higher incidence of retained common bile duct stones (16.7% vs. 9.7%) and a trend towards more post-ERCP complications.

    Conclusions:

    • Common bile duct cannulation is more challenging in patients with periampullary diverticulum.
    • Periampullary diverticulum is linked to an increased risk of retained common bile duct stones.
    • These factors may contribute to a higher post-ERCP complication rate in patients with periampullary diverticulum.
