Science (New York, N.Y.)·2019

Area of Science:

  • Pediatric Rheumatology
  • Pediatric Vasculitis
  • Inflammatory Syndromes

Background:

  • Polyarteritis nodosa (PAN) is a rare systemic vasculitis.
  • Acute cholecystitis is an uncommon presentation of PAN in adults.
  • Pediatric cases of PAN presenting with acute cholecystitis are exceptionally rare.

Observation:

  • Two pediatric patients presented with acute cholecystitis.
  • Both patients exhibited persistent inflammatory syndrome.
  • Antineutrophil cytoplasm antibodies were positive in both cases.

Findings:

  • Vasculitis with fibrinoid necrosis was confirmed in both patients.
  • The diagnosis of polyarteritis nodosa was established in these pediatric cases.
  • Cholecystectomy facilitated the diagnosis in the context of acute cholecystitis.

Related Experiment Videos

Implications:

  • This highlights the importance of considering PAN in children with acute cholecystitis.
  • Early diagnosis of PAN is crucial for effective management.
  • Surgical intervention, such as cholecystectomy, can be diagnostically valuable in specific pediatric cases.
