Summary

Aging men with lower testosterone levels experience reduced erectile function. Dihydrotestosterone (DHT) treatment improved nocturnal erectile function in these men, suggesting a role for androgens in maintaining erectile health in older age.

Area of Science:

  • Andrology
  • Gerontology
  • Reproductive Endocrinology

Background:

  • Erectile function naturally declines with age, impacting many older men.
  • Biochemical hypogonadism, defined as low androgen levels, is prevalent in men over 60.
  • The relationship between androgen levels and erectile function in aging men remains unclear.

Purpose of the Study:

  • To investigate the association between circulating androgens and sexual activity in healthy older men without erectile dysfunction.
  • To compare hormonal profiles and nocturnal erectile function between aging men and younger controls.

Main Methods:

  • Hormonal assessment including total testosterone, free testosterone, DHT, FSH, and LH.
  • Nocturnal penile tumescence monitoring over two consecutive nights.

Related Experiment Videos

  • Comparison of data between a group of 40 healthy older men (mean age 68) and 30 young controls (mean age 33).
  • Main Results:

    • Older men exhibited significantly reduced frequency, duration, and degree of nocturnal erectile episodes compared to younger men.
    • These erectile function declines were more pronounced in older men with lower testosterone levels.
    • DHT treatment improved nocturnal penile tumescence parameters in hypotestosteronemic older men, though they remained lower than controls.

    Conclusions:

    • Hypoandrogenism in aging men may negatively impact erectile mechanisms, even with normal sexual activity.
    • Low androgen levels could be a significant co-factor in the development of age-related erectile dysfunction.
    • Androgen replacement therapy warrants further investigation for improving erectile function in aging men.
