Summary

Multiple sclerosis can exceptionally present with severe, proliferative retinal vasculitis, leading to vision loss. Early diagnosis is crucial as the visual prognosis for these rare retinal complications remains uncertain.

Background:

  • Retinal periphlebitis is a known, though infrequent, manifestation of multiple sclerosis (MS).
  • Proliferative retinopathy is an exceptionally rare complication of MS.

Observation:

  • A case of a 28-year-old woman with bilateral, proliferative, retinal vasculitis and recurrent vitreous hemorrhages associated with MS is presented.
  • The patient initially experienced unilateral central venous thrombosis with ischemic and proliferative changes.
  • Treatment included Interferon for MS, pan-retinal laser photocoagulation, and vitrectomy for retinal complications.

Findings:

  • The patient developed bilateral epiretinal membranes and chronic macular edema post-treatment.
  • Visual acuity remained significantly impaired in both eyes after 2.5 years of follow-up.
