Summary

Congenital abdominal aortic aneurysm (AAA) is rare in infants. This study details the first successful repair of a neonatal AAA using an antigen-reduced cryopreserved allograft, showing excellent early results.

The effects of the addition of a pediatric surgery fellow on the operative experience of the general surgery resident.

Background:

  • Abdominal aortic aneurysm (AAA) is rare in pediatric patients, often linked to infection, trauma, vasculitis, or genetic disorders.
  • Congenital neonatal AAA is exceptionally rare, with limited treatment options previously reported, primarily synthetic grafts or aneurysmorrhaphy.

Observation:

  • A 4-month-old infant girl presented with a 6 cm congenital infrarenal AAA.
  • The infant underwent surgical repair using a 5 mm cryopreserved allograft pretreated with an antigen reduction process (SynerGraft).

Findings:

  • The surgical repair was successful, with an unremarkable postoperative course.
  • Excellent lower extremity pulses and duplex ultrasound flow were maintained at 14-month follow-up.
  • The patient remained negative for panel reactive antibodies against class I alloantigens.

  • Antigen-reduced allografts represent a viable option for congenital AAA repair in infants.
  • This approach may reduce allograft degeneration and stenosis, accommodating pediatric growth.
