Background:

  • Pericardial tumors are rare, often presenting with nonspecific symptoms.
  • Synovial sarcoma is an uncommon soft tissue malignancy, rarely originating in the pericardium.

Observation:

  • A 29-year-old male presented with acute breathlessness.
  • Imaging revealed a massive tumor on the left lateral pericardial surface.

Findings:

  • Surgical excision was performed, and histopathology confirmed synovial sarcoma.
  • Postoperative radiation and chemotherapy were administered.

Implications:

  • The case highlights the diagnostic and therapeutic challenges of primary pericardial synovial sarcoma.
