Summary

A young man experienced acute breathlessness due to a massive pericardial tumor, diagnosed as synovial sarcoma. Despite surgery and adjuvant therapy, local recurrence occurred, highlighting challenges in managing this rare cardiac tumor.

Background:

Pericardial tumors are rare, often presenting with nonspecific symptoms.
Synovial sarcoma is an uncommon soft tissue malignancy, rarely originating in the pericardium.

Observation:

A 29-year-old male presented with acute breathlessness.
Imaging revealed a massive tumor on the left lateral pericardial surface.

Findings:

Surgical excision was performed, and histopathology confirmed synovial sarcoma.
Postoperative radiation and chemotherapy were administered.

Implications:

The case highlights the diagnostic and therapeutic challenges of primary pericardial synovial sarcoma.
