Area of Science:

  • Medical Education
  • Academic Medicine
  • Healthcare Management

Background:

  • Medical schools have transformed into academic medical centers (AMCs) with a growing emphasis on research and clinical revenue.
  • This shift has sometimes overshadowed the primary mission of educating future physicians.
  • Existing departmental structures may not align with the evolving needs of AMCs.

Purpose of the Study:

  • To analyze the impact of AMCs' evolving priorities on medical education.
  • To propose strategies for re-establishing the educational mission within AMCs.
  • To suggest frameworks for better aligning organizational structures with institutional missions.

Main Methods:

  • Review of the organizational and governance structures within AMCs.
  • Analysis of the influence of revenue streams on institutional priorities.
  • Proposal of mission-based management and budgeting frameworks.
  • Recommendations for structural and operational changes.

Main Results:

  • AMCs have achieved success in research and clinical care but often at the expense of education.
  • A misalignment exists between the governance structures of AMCs and their core missions.
  • Current structures may not adequately support the educational mission.

Conclusions:

  • Medical schools must actively distinguish core missions from business activities.
  • Implementing mission-based management and budgeting is crucial for success.
  • Specific reforms, including task forces, core teaching faculty, and education centers, are recommended to revitalize the educational mission.
